Male child 12 yo suffered from long-term constipation leading to lost of appetite and weight. Ultrasound detected a thin wall cyst - 70x64x71mm - in his pelvis, in front of the sacral region. Its posterior wall was connected with the sacrococcygial part of the vertebra and the anterior face of S5 was not observed. This mass was full of homogenous liquid, no sludge, nor vegetation, nor septa and slightly getting bigger in upright position. On MRI, this cyst began from the posterior sacral canal, left side of S4-5, emitted a signal equal to that of liquid, with T1W1 a low signal equal to that of cebrerospinal liquid, decreased signal with T2 Plair. As a result, the cyst slightly push the rectum toward the right side. Bones and cartilageneous discs were intact. Surgical operation revealed an extradural Tarlov cyst. This cyst started from posterior of S1 with a long stalk then turned to anterior of the sacrococcygial part at S4-5.
